3.  APPS Internet "BASELINE" Audits:  The price of this service starts at $1500.00 per medical service provider.

     Upon Internet instructions (including payment directions and a signed Business Partner's Agreement), you the medical service provider would send an e-mail file to APPS (at appsservices@aol.com) holding a scanned, random selection of your E&M services, with at least five examples from each main E&M service section (99201-99205, 99211-99215, 99241-99245, 99251-99255), unless you provide E&M services other than from these sections, for a total of at least 35 total services reviewed.  You would select this sampling from your payment EOB's, with payment dates within the past six months.  The audit sampling e-mail file would include a copy of your original payment EOB, charge ticket/superbill, progress note, and a reprinting to white paper of the originally submitted CMS (HCFA) claim form.  The audit would take place by a Certified Medical Coder, and within a 10-day period you would receive a written report by APPS validating your independent external audit.
